1986年。ドイツで働く日本人医師の天馬賢三は、将来を嘱望され、婚約者のエヴァ・ハイネマンとともに、一見順調な日々を送っていた。しかし、実際は院長のエヴァ・ハイネマンの父親（院長）から利用され、意に沿わない仕事ばかりをさせられていた。ある日、院長の命令を無視して、重傷を負ったヨハン・リーベルトの手術を行った賢三は、手術には成功したものの、出世コースからは外され、エヴァとの婚約も破棄されてしまう。それを悔しく思いつつも、ようやく自分らしい仕事ができると考えた賢三だったが、院長らが突如ヨハンの手によって殺害される。ヨハンが恩人である賢三のために院長らを殺したこと、さらに、他の多くの殺人事件にも関与していることを知った賢三は、強い責任を感じ、「ヨハンの命を救ったことで、自分は凶悪犯を世に放ってしまったのではないか」と苦悩する。そして賢三は医師をやめ、たった1人でヨハンを追うことを決意する。

The year is 1986. Tenma Kenzo, a Japanese doctor working in Germany, is expected to have a bright future, and at first glance, he seems to be living a smooth life with his fiancée, Eva Heinemann. However, in reality, he is being used by Eva Heinemann’s father (the director), who is the director of the hospital, and is forced to do work that is not to his liking. One day, Kenzo ignores the director’s orders and operates on the seriously injured Johan Liebert. Although the operation is successful, he is removed from the promotion track and his engagement to Eva is broken off. Although Kenzo feels regretful about this, he thinks that he can finally do the work that suits him, but the director and others are suddenly murdered by Johan. Kenzo feels a strong sense of responsibility when he finds out that Johan killed the director and others for his benefactor, Kenzo, and is also involved in many other murders. He is tormented by the thought that “by saving Johan’s life, I may have released a violent criminal into the world.” Kenzo then decides to quit his job as a doctor and pursue Johan alone.
